Integrated Medication Management
While medications are commonly used to treat chronic conditions, use of multiple medications may lead to adverse events, a top cause of avoidable hospitalizations and ER visits. The Integrated Medication Management (IMM) capabilities in Jiva’s Clinical Intelligence suite systematically identify medication-related concerns using evidence-based guidelines and formulary intervention opportunities. Leveraging a preferred drug list (PDL) and IMM ruleset, users can initiate appropriate interventions within the care plan or at the member level.
Preferred Drug List
With the PDL, users create and manage formulary statuses and edits for medications for multiple lines of business. The PDL uses the Medi-Span drug database to identify therapeutic drug classes and medications that are managed in the formulary. Information entered in PDL is used to identify intervention opportunities to increase formulary compliance and optimize drug utilization.
Rules in Jiva that identify medication-related issues populate a medication issue list that is presented to the user for resolution via a plan of care within the member’s medication issue list. Examples of issues include:
- Drug interactions
- Medication safety
- Preferred drug use
- Treatment adherence
