Plano, Texas – November 16, 2017 – A large health plan in the Pacific Northwest has upgraded to Jiva™ 6.1, the end-to-end population health management (PHM) platform from ZeOmega®, a market leader in population health management solutions. Jiva 6.1 will help improve care coordination by enabling the seamless exchange of health information and facilitating timely communication between the health plan, its members, and its physicians.
A longstanding client of ZeOmega, the health plan had previously licensed, and locally installed, an earlier version of the Jiva platform.
