By Sandra Hewett, Director of care management at Community Health Plan The continued use of manual processes raised a real concern for program changes and tracking with newer and more robust state and federal regulations. Community Health Plan (CHP) in Washington faced several challenges that needed to be addressed in order to achieve its goal of deploying a patient-centric delegated care-management system. For years, care managers at CHP used multiple, disparate systems for case management (CM), disease management (DM) and utilization management (UM) – resulting in information silos. Without an integrated system, there was a lack of evidence-based clinical pathways to ensure consistent care planning and management of patients. As a result, data collection was not robust, tracking was inadequate and reporting was not streamlined. In keeping with its vision and to meet organizational needs, CHP decided to deploy a patient-centric care-management system that emphasized collaboration between care managers, members and physicians, supporting the medical home philosophy. CHP envisioned the future deployment of a delegated care-management service model, where the primary physicians in its community health centers would actively participate in and/or lead patient care management. "The manual systems – some paper-based – created inefficiencies, lowered data integrity and decreased coordination of care for the patient," says Sandra Hewett, director of care management at CHP. CHP was created in 1992 by a group of community health centers across Washington state. Overall, CHP offers access to locally delivered treatment and services for more than 240,000 individuals. "We realized that if we continued with a manual system, we would face potential risk with data integrity issues and the inability to produce comprehensive reporting. In preparing for increased regulatory requirements and accreditation plans, we concluded that increased integration between UM, CM and DM were imperative." CHP was also cognizant of the need to meet the new HIPAA regulations, and the need for robust tracking and reporting capabilities when it pursued National Committee for Quality Assurance (NCQA) accreditation in 2010. Recruitment takes a role Given the nationwide shortage of nurses, CHP, like other organizations, also faced the challenges of recruiting and retaining qualified RN staff. Moreover, the lack of efficient integrated information systems further exacerbated the difficultly in recruiting, training and retaining nursing staff, while further adding to the annual operating costs. These issues loomed large as the health plan looked toward future expansion, which would require better connection and integration. The CHP team concluded the organization would need a patient-centric care-management platform that would: integrate its work flows, such as UM, CM, DM, appeals and grievances, across all lines of business; be configurable to enable CHP to meet not only current, but also future business requirements; provide robust data collection, tracking and reporting; increase efficiencies and reduce errors through automation; enable CHP to comply with the various regulatory and accreditation requirements; and be intuitive and easy to learn and use. After a thorough evaluation of leading solutions, CHP selected Jiva, an integrated care-management software solution from ZeOmega. Hewett was charged with overseeing the implementation. In 2008, CHP deployed Jiva’s integrated UM, CM, DM, appeals and grievances work flows centered on an assessment-driven care plan. "We used Jiva to aggregate data such as claims and pharmacy from other systems and integrate evidence-based guidelines into its decision-making processes, as well as influencing case-finding activities," says Hewett. In a second phase in 2009, CHP introduced additional features that fine-tuned its system and optimized work flow. Through Jiva, CHP was able to integrate information across claims, labs, pharmacy and other internal systems and across different work flows. CHP staff is now able to see a consolidated view of patient information. CHP also achieved its goal of deploying a patient-centric care-management solution, enabling care managers, providers and other stakeholders access to patient information from multiple points. This allowed clinicians to access and complete patient information with real-time data. Through Jiva, they were able to use integrated evidence-based guidelines during the decision-making process, thereby ensuring consistency in the application of benefit determinations. As a result, CHP continues to see improved patient care and outcomes. According to Hewett, Jiva enabled CHP to realize significant efficiencies through its streamlined work flows, business rules-driven processes, and automated/scheduled reporting and correspondence, as well as other capabilities such as alerts and schedulers. Improved efficiency, cost savings Prior to deployment, CHP performed a detailed analysis that established expected cost savings and efficiency gains from deploying the software. According to Hewett, an increase in case-management efficiency of 30 percent has yielded a corresponding increase in referrals to case management. This has resulted in more proactive management of patients’ needs, she says. Utilization management also saw an increase in productivity and improvements in turnaround times, Hewett reports, as CHP increased its utilization-management efficiencies with a 47 percent increase in its caseload per full-time equivalents. CHP realized further gains in February 2009 when it implemented batch integration between its external claims processor and Jiva. The additional integration allowed CHP to further increase the number of cases handled by case managers by 25 percent. A similar percentage increase was noted in the number of UM reviews completed by the CHP staff. CHP was able to integrate information across claims, labs, pharmacy and other internal systems and across different work flows. CHP staff is now able to see a consolidated view of patient information. "We realized additional benefits in areas of employee productivity and expanded market presence," Hewett says. "Because Jiva is a Web-based integrated care-management platform, we are now able to provide secure access for staff to CM, DM, UM and other patient information through our Web browser from a remote location. This has created remote work opportunities for care-management staff, thereby improving employee satisfaction." This remote-access capability also led to the launch of a satellite CHP office in Spokane, expanding the health plan’s customer service to both members and providers. Additionally, the satellite office strengthened CHP’s already tight linkage to its community health centers, Hewett says, thus strengthening a key strategic goal of the health plan. "We were able to transform our fragmented care-management environment into a proactive, connected healthcare ecosystem," she says. "Real-time access to member health-risk assessments, clinical pathways and other patient information enabled us to more quickly identify at-risk members and create customized care plans and management strategies, reducing medical costs and enhancing outcomes." CHP plans to offer its vision of a delegated care-management service model at its community health centers by deploying the Jiva Provider Portal in early 2010. By Nandini Rangaswamy, CFO and executive VP, ZeOmega Congress and policymakers across the country are considering several means to improve quality of care and control spiraling costs as part of the raging debate on healthcare reform. Although care management has not been explicitly discussed as the means to effect change, it will emerge in 2010 and beyond as one of the most promising and effective ways to deliver on the goals of healthcare reform. Care management as a concept, of course, is not new. What is new is the shift in approach and the technologies that make it feasible. There is emphasis on integration of information, collaboration between payers and providers, and communication with members. In the last few years there has been a push towards integrated utilization, case and disease management. Although payers have realized some benefits from this integration through improved efficiency and reduced errors, they recognize that there are further gains to be realized by proactively identifying high risk members, engaging them at the right time and by eliminating gaps in care. Payers can glean vital information on a member’s health by integrating and analyzing claims, pharmacy, lab and diagnostic data housed in disparate systems. This information can be leveraged to drive rules-based care management workflows. Tightly integrating the three components — analytics, rules and workflows — can truly revolutionize care management and shift the focus from collecting data to addressing quality issues. A few pioneers have already launched initiatives to do this. Care analytics is helping these payers identify high risk members and gaps in care. High risk members are automatically routed to outreach teams for appropriate engagement. Gaps in care are flagged as alerts in the nurses’ dashboard for appropriate intervention (for e.g. a patient failing to refill two prescriptions triggering an alert to the nurse to call the patient and determine the cause). Tight integration between care analytics and rules-driven care management workflows is enabling precise and timely interventions resulting in markedly better outcomes and savings. As Medicare, employers and other purchasers hold payers more accountable for quality and costs, health plans will increasingly rely on integrated solutions to demonstrate value to clients. There is now a general consensus on the correlation between provider collaboration and efficacy of care management. For example, care payers and providers alike are seriously considering management models such as patient centric medical home. Integrated provider portals, inbuilt messaging, discussion boards and other tools that facilitate collaboration will become critical for deploying those models. Another area of importance will be technologies that facilitate health information exchange. Whether it is integration with EMRs, or personal health records or RHIOs, these technologies will play a critical role in determining how information flows back and forth between providers, payers and other third party providers. Finally, technologies that enable multi-channel communications based on member and provider preferences will gain importance as payers seek to disseminate the right information to the right person at the right time through the preferred mode of communication. Data integration and data mining help identify, evaluate and stratify members for care management, said Judith Frampton, HPHC's vice president of medical management. With Jiva, HPHC can create interfaces that aggregate data from multiple department systems and design algorithms that build registries of at-risk members for specific conditions, she said. Payers are bringing their care management programs in-house to better manage their members' conditions in one place and with one point of contact, said Sam Rangaswamy, ZeOmega's CEO. It requires IT solutions to integrate all programs across the care continuum. Jiva's care analytics identifies and stratifies members based on their clinical conditions rather than actuarial models, he said. "We provide better accuracy on who to focus on, not from a cost standpoint but from a clinical standpoint," he said. Social networking is still relatively new for payers, said Lynne Dunbrack, program director for Health Industry Insights. To stay ahead of the curve, payers need to adopt social networking technologies and services, commonly referred to as Health 2.0, to engage members, provide differentiation from competitors, and establish and sponsor a community for payers' constituents, among other things, she said. HPHC is doing just that with its sponsorship of the community site www.wellthen.org , said Frampton. Health plans say they want to see healthier patients and more efficient physicians, and that offering education and guidelines is one way to do that. The skeptical audience, however, wonders if it's all about money. Health plans are moving away from care and into "care management." Knowing that they don't inspire trust, but that physicians do, insurers want to make physicians messengers for health plans, improving members' health to save money. But they also want to prompt doctors to change their own behavior for even more savings to the plan. "Health plans are realizing they have to work with the trusted brand of the physician," said Dennis Schmuland, MD, a family physician who is the health plans industry solutions director for the U.S. Health and Life Sciences Group at Microsoft. The difference between care and care management, proponents say, lies in enhancing the role of the physician -- at worst in a superficial sense, and at best in a way that allows the physician to make good decisions without interference. At the annual meeting of America's Health Insurance Plans, held in San Diego in June, some of the flashiest, biggest booths -- one offering martinis, another offering California wine -- were hosted by firms vying to sell health plans the perfect care-management tool. All promised a means to gently, effectively use physicians to change patient behavior in a way that doesn't alienate either the patient or physician, and results in savings for the insurer. Some vendors, such as Emeryville, Calif.-based MedeAnalytics, offer software packages for hospitals and health plans. Other firms have integrated care-management prompts into existing products. Boston-based American Well, for instance, has integrated prompts into its portal for online medical visits. "What we see in the market is a proper evolution: You have to keep the doctor in the center," said American Well President and Chief Executive Officer Roy Schoenberg, MD. "Patients viscerally say, 'I need to see a doctor.' " In many cases, care-management systems can be integrated into a health plan's Web page and are automated. So when staff at the front desk run an eligibility check, the system produces reminders and suggestions, such as, "This patient's record shows some signs that she has a history of depression. Consider giving her a depression questionnaire as part of this visit," or, "This patient is due for a mammogram. Please schedule an appointment soon." Those messages could show up in an e-mail, a text message or a window on a doctor's handheld computer. "It's kind of catching the patient at the moment of truth," said Ted Ryan, vice president of sales for ZeOmega, a Frisco, Texas-based care-management system company. "You've got the data and got the patient in front of you. Before, it would have been a letter after the fact, or the ability to extract that data and push it back into clinic just wasn't there -- that was a daylong project." Doctor skepticism But physicians may not be easily convinced that health plans are interested in "care management" for the right reasons, or that health plans can improve the system. "I think it's a waste of money and not helpful," said Joseph W. Stubbs, MD, an internist from Albany, Ga., who is president of the American College of Physicians. Care-management systems can be integrated into a health plan's Web page. "So many times, their information is erroneous and irrelevant, because they don't have access to the clinical record," he said. "I can't tell you how many pieces of paper I have that say have you considered this or that." It would make more sense, he said, to pay doctors directly to manage care rather than hiring a third company to deliver messages from the health plan to the patient. Short of that, he said, for any health plan communication to be effective, it should be initiated by the physician, completed electronically and linked to a clinical record, not just claims data. Health plans and vendors of care-management systems said they know the problems. Doctors are overwhelmed with information, most of which comes at the wrong time, the wrong way or both. Rather than simply using a preapproval requirement, "when a physician does make a request for advanced imaging or prescription, why not deliver the evidence-based guidelines side by side with the approval?" Dr. Schmuland asked. There is also the credibility problem. Doctors don't always trust clinical information handed to them by health plans, so the health plans need a third party to provide objective clinical evidence that will sway doctors to do the right things. That's where the care-management companies say they come in. They try to stress to doctors that they're not all about money. Instead, they pitch how their systems focus on quality measurement based on established standards. Some also use systems that integrate the practice record. For example, MEDecision, a subsidiary of nonprofit Blues plan holder Health Care Service Corp., uses electronic health records as touch points for advising physicians, hospitals, health plans and patients. It identifies "gaps in care" and passes that information on to doctors in a nonintrusive way. Scott Storrer, president and chief operating officer for MEDecision, said many physicians in focus groups hosted by the company said they would pay for the system themselves because they found it so useful. Care-management companies also say they could solve one of health insurers' big quandaries: how to use the vast amount of information they hold -- claims data about members' hospital stays, visits to the doctor, prescription drugs -- to cut costs and improve care. Over the past five years, some health plans have tried sending the information and tools they own straight to members. The idea is that motivated patients spending their own money will search for value, educate themselves and act in their own best interests while driving quality and efficiency. It's still in play, but it hasn't worked out as quickly or as well as some had hoped. Consumers have proven difficult to reach, they don't have the information they want, and, despite sometimes having good information, they still don't always make the best choices. "I think there's less belief that it really is just a health-plan-to-consumer interaction that's going to make the difference," Dr. Schmuland said. "There's recognition across the industry that health care is a collaborative team process, and that to really have the long-term impact in reducing chronic disease and improve health, that requires more than just giving information to the consumer." Not insignificantly, care-management companies say health plans are willing to pay doctors for their time and attention, either under a capitated payment system or a fee-for-service arrangement as part of pay-for-performance programs. Proponents say that under a health plan's care-management system integrated into its existing portal, doctors would be prompted at the time of care to follow pay-for-performance guidelines. Tracking success would be simpler, and the clinical rationale for the pay-for-performance incentives would be delivered to the doctor at the time of care. "As annoying as pay-for-performance can be, physicians know it's coming, so the more opportunity they can have to get access to data [that supports payments], the better," said Terry Fouts, MD, chief medical officer for MedeAnalytics. Beyond paying for doctors to follow health plans' advice, those in the field said health plans also are figuring out that much of their success depends on the tone of their communications. The key to getting doctor buy-in is in how the message is framed, said Matthew Zubiller, business leader for advanced diagnostics management for McKesson, which owns InterQual, one of the oldest care-management tools. "Physicians don't mind being asked questions, but not stupid questions," he said. "This is about engaging the health care provider in a different way."
