Washington State Health Information Exchange Adds Jiva Population Health Management Solution to Deliver Comprehensive Care Across Regional Network
Integration to Increase Efficiencies and Outcomes by Bringing Actionable Patient Data to the Point of Care
PLANO, Tex. July 13, 2016 – ZeOmega® Inc., a market leader in population health management solutions, today announced that the Medical Information Network – North Sound (MIN-NS), an independent community-based health information exchange (HIE) in Mount Vernon, WA, has added Jiva™ Population Health Management (PHM) solution to its existing Jiva HIE capabilities. MIN-NS will leverage Jiva PHM to support patient-centered, outcome-focused care management across its member network of over 20 clinical facilities in Skagit and other surrounding counties.
For more than five years, MIN-NS has leveraged Jiva HIE to enable information exchange among its network of hospitals, clinics, labs, long-term care facilities, and service providers. By adding Jiva PHM, MIN-NS will provide these organizations with a more holistic solution that offers deeper insight into individuals and populations, and advanced resources for more impactful interventions and process efficiency improvements. Specifically, MIN-NS will leverage Jiva PHM to:
- Integrate clinical and non-clinical data from admissions, discharges, and transfers systems; claims and billing platforms; and electronic health records, among others
- Deliver actionable intelligence directly into the workflows of care team members
- Create a single electronic patient medical record
- Engage, educate, and empower individuals and their care team
- Support Accountable Care Network (ACN) participants

MIN-NS has also extended its available Jiva HIE resources by adding two new capabilities. The first, Care Transitions, helps coordinate care continuity as individuals move across care settings. The other, eHealth Exchange, will enable MIN-NS to connect with a larger nationwide HIE involving federal government agencies and other healthcare organizations – which is central to its ultimate vision. The rollout of both new Jiva HIE functionalities has already begun and is expected to be finished later this year, as is the addition of Jiva PHM.
