Plano, Texas – December 7, 2017 – A national integrated delivery network (IDN) and longstanding ZeOmega® client recently upgraded to JivaTM 6.1, the industry leading end-to-end population health management (PHM) platform. Now, the IDN can more eﬀiciently manage patient data, coordinate care, and identify opportunities to improve performance.
Major growth across the IDN prompted the upgrade, encouraging the achievement of high-value care across disparate organizations, workflows, and health information systems. As the IDN expanded its regional footprint and engaged with a growing number of stakeholders across the healthcare continuum, accessing, exchanging, and reconciling patient data from multiple sources in a secure manner became increasingly diﬀicult.
