A board-certified internist, Dr. Nash is internationally recognized for his work in public accountability for outcomes, physician leadership development, and quality-of-care improvement. Repeatedly named to Modern Healthcare’s list of Most Powerful Persons in Healthcare, his national activities cover a wide scope. Dr. Nash is a principal faculty member for quality of care programming for the American Association for Physician Leadership (AAPL). He serves on the NQF Task Force on Improving Population Health and the John M. Eisenberg Award Committee for The Joint Commission. He also is a founding member of the AAMC-IQ Steering Committee, the group charged with infusing the tenets of quality and safety into medical education. Dr. Nash was also recently appointed to the Executive Research Advisory Council (ERAC) of the Pharmaceutical Quality Alliance (PQA) in Washington, DC.

Marla Pantano, FSA, MAAA
Marla is a seasoned healthcare executive with extensive experience in population health, data analytics, and actuarial science. Currently, she is the President of CarePartners of Connecticut, a not-for-profit Medicare insurance company formed by Hartford HealthCare and Tufts Health Plan. She oversees all aspects of the health plan’s growth and performance, including strategy, marketing and sales, quality, provider network and medical cost management.
Formerly Marla was at Agilon Health, and oversaw data solutions, analytics, actuarial and enterprise clinical operations. Additionally, she has served as Chief Actuary and VP of Population Analytics for one of the nation’s largest hospital systems and headed the Medical Analytics and Healthcare Reform organizations for a leading national health insurer.
Through her roles, Marla has partnered with providers to drive transformation in healthcare delivery by combining strong data analysis with clinical knowledge to improve the quality and efficiency of healthcare delivery as well as improve provider and member experience.

ONC Health IT – Costs and Limitations Disclosures
Jiva 6.1-2015 Edition Health IT Modular Certification-Direct project, Edge Protocol, and XDR/XDM
Jiva version 6.1 attained 2015 Edition Health IT Modular Certification from the ICSA Labs ONC Health IT Certification Program. Certification was granted based on rigorous and thorough testing by ICSA Labs, an Accredited Test Lab (ATL). Jiva version 6.1 was certified on January 6, 2020 by ICSA Labs, an Office of the National Coordinator-Authorized Certification Body (ONC-ACB) and is compliant in accordance with applicable criteria adopted by the Secretary of Health and Human Services (HHS).

ICSA Labs, an independent division of Verizon, has been providing credible, independent, third-party product assurance for end-users and enterprises since 1989. ICSA Labs provides third-party testing and certification of security and health IT products, as well as network-connected devices, to measure product compliance, reliability and performance for most of the world’s top technology vendors. Jiva 6.1’s certification number is 190017R00.

There are no known limitations that a user may encounter in the course of implementing and using Jiva 6.1.
NCQA PHM in Health Plan Accreditation Prevalidation
Jiva became a prevalidated solution in 2018 when NCQA’s PHM Prevalidation program began and its PHM Prevalidated status continues, uninterrupted, through 2021.
With this Prevalidation, our clients benefit from reduced burden for Health Plan Accreditation (HPA) as well as Population Health Program (PHP), Managed Behavioral Health Organization (MBHO), or Case Management (CM) accreditation elements designated as eligible for automatic credit or plan support. Because Jiva has been rigorously tested and proven to ease administrative workload, organizations invested in our solutions will regain precious time to focus on patient-centric care and the improvement of overall health outcomes.
Jiva Achieves HEDIS 2020 Allowable Adjustment Certification.
ZeOmega has received certification for its population health management platform, Jiva, for NCQA HEDIS Allowable Adjustments Certified MeasuresSM 2020. NCQA’s HEDIS Allowable Adjustments (AA) Measure Certification is precise, automated testing that verifies compliance with the Rules for Allowable Adjustments of HEDIS.
ZeOmega selected thirty measures that reflect the broad range of coverage provided by Jiva for identification and management of population health care issues across the continuum, including age-appropriate health and wellness indicators for preventive health and screenings, age- and condition-related measures such as Care for Older Adults, Controlling High Blood Pressure and Comprehensive Diabetes Care, as well as multiple behavioral health measures.
Using Jiva’s Clinical Quality Dashboards and Gaps-in-Care permits our clients to proactively monitor performance and provide needed interventions to enhance effectiveness and quality of care for their members. The rigorous automated testing for successful certification further assures accuracy of reporting measures that are based on complex software logic.
This is ZeOmega’s report on our service organization’s system and the suitability of the design and operating effectiveness of the controls. It reports on controls at a service organization relevant to security, availability and confidentiality. SOC 2® reports are examination engagements performed by a service auditor (CPA) in accordance with AT Section 101, Attest Engagements, of SSAEs (AICPA, Professional Standards) using the predefined criteria in TSP section 100, Trust Services Principles, Criteria, and Illustrations for Security, Availability and Confidentiality. SOC 2 reports specifically address the following key system attributes:
Security – The system is protected against unauthorized access (both physical and logical)
Availability – The system is available for operation and use as committed or agreed
Confidentiality – Information designated as confidential is protected as committed or agreed
The Sequoia Project—an independent advocate for the implementation of a secure, interoperable, nationwide health information exchange (HIE)—has validated Jiva HIE Connect 6.0.2 for use with the eHealth Exchange.
The Sequoia Project developed the eHealth Exchange Product Testing Program to provide a high degree of assurance that participating systems conform to eHealth Exchange Performance Specifications and can interoperate with other systems without error or further customization.
Having successfully passed the eHealth Exchange Product Testing Program’s rigorous conformance test, Jiva HIE Connect can now exchange data with the largest HIE in the United States.
DirectTrust HISP Accreditation
The Accreditation Program recognizes excellence in health data processing and transactions, and ensures compliance with industry-established standards, HIPAA regulations and the Direct Project. Launched in March 2010 as a part of the Nationwide Health Information Network, the Direct Project was created to specify a simple, secure, scalable, standards-based way for participants to send authenticated, encrypted health information directly to known, trusted recipients over the Internet.
DirectTrust participating organizations are evaluated in the areas of privacy, security and confidentiality; technical performance; business practices and organizational resources as they relate to Directed exchange participants. Additionally, their process of managing and transferring protected health information is assessed and determined to meet or exceed all DirectTrust criteria and industry standards. Successful Accreditation Program completion demonstrates organizations’ adherence to strict standards and participation in the comprehensive, objective evaluation of their business.
